ILLSIDE MANOR's Long Term Home Health Care Program is one of the outstanding services of the Hillside Manor Healthcare Network.

Also known as "Nursing Homes Without Walls" or the Lombardi Program, Hillside Manor's Long Term Home Healthcare Program provides home care services for persons who need assistance to maintain their independence in their home.

Are you or a loved one disabled or chronically ill? Are you or your loved one eligible for admission into a nursing home, but would prefer to remain at home? Hillside Manor's Long Term Home Health Care Program is licensed to provide long term home care services in the borough of Queens. The program offers professional services such as nursing, physical therapy, occupational therapy, speech therapy, medical social work, registered dietician and respiratory therapy. In addition home health aides and or personal care workers are available to provide assistance with activities of daily living.

If you or your loved one:

• have Medicaid or are eligible for Medicaid
• are under the care of a physician
• have a need for coordinated services such as nursing, physical therapy, occupational therapy, speech therapy, medical social work, registered dietician and respiratory therapy
• have a chronic medical condition or are disabled
• reside in Queens, New York

Coordination of Services to Include:

• Registered Nurse in home visit to observe/assess the patient's clinical status, instruct in the plan of care and provide treatment as ordered by the physician.
• Physical Therapy to evaluate the need for intervention and to establish a plan of treatment.
• Occupational Therapy to evaluate the need for intervention and to establish a plan of treatment.
• Speech Language Therapy to evaluate the need for intervention and to establish a plan of treatment.
• Home Health Aide/Personal Care worker up to 42 hours per week to assist with personal care and home management.
• Medical supplies and equipment

Waiver Services:

• Nutritional Counseling
• Respiratory Therapy
• Medical Social Work Counseling
• Home Maintenance Tasks
• Housing Assistance
• Moving Assistance
• Respite Care
• Social Day Care
• Home Improvement
• Social Transportation
• Personal Emergency Response System

Provision of transportation via ambulette for medial appointments and or ambulance for emergency services.
